F-1 New Student Check-in Form

Welcome to the Rice University Intensive English Program.

SEVIS requires new F-1 students to check in with their school on arrival. You may do so by completing this form. You will be asked to provide your U.S. Address, and phone number along with copies of your signed I-20, passport ID page, and F-1 Visa. We also require a copy of your I-94. You may upload the copy of the I-94 on this form or give us permission on the form to download the I-94 for you.
